Office Policies

Prescription Renewal Policy

Medication refill requests are accepted during regular office hours. When requesting medication refills, please be sure to allow 48 hours (two business days) notice. Please contact your pharmacy directly and they will notify us of your request. If you are unable to do this, please provide the spelling of your name and your date of birth as well as the name of the medication, dosage, how often you take it, the pharmacy name, location, and phone number, and a phone number where you can be reached during the day if the nurse has a question. Check with your pharmacy before picking up your prescription.

Telephone Calls

Test results: You will be contacted if diagnostic results require a change in therapy, or other intervention ordered by you practitioner. Normal or borderline results, or results that require a detailed explanation will be discussed at your next office visit.

Emergencies

If you think you are experiencing a potentially life threatening situation, Please call 911.

For urgent problems after hours, weekends, and on holidays, call: (757)-875-9364 and ask for the doctor on call. All other types of emergencies should be directed to your primary care physician.

Medical Records

Due to the volume of requests for medical records, please note that we require 7-10 business days for processing. As a courtesy to our patients, we will provide medical records to other health care providers with proper authorization from the patient. Patient who request a copy of their medical records will be charged a nominal fee.

Administrative Request

We will complete disability forms (only if we put you out on disability), Family Medical Leave, and other administrative forms for a nominal fee, which is payable in advance. Requests are processed in the order that they are received and may take up to 10-14 business days.

Privacy Practices

Our Notice of Privacy Practice describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
