Thank you for choosing Arthritis and Rheumatic Diseases for you Rheumatology care. We are committed to the success of your medical management. We are certain that you already understand that prompt payment of any balances due on your account is part of this treatment and care. Because there are literally hundreds of health plan options, it is commonly accepted that understanding and clarifying all benefits and exclusions under your health plan is ultimately your responsibility. As a courtesy to you, our office will file insurance claims for you. We participate with most major insurance companies and accept payment on your account by cash, check, VISA, MasterCard, Discover, and American Express.

Co-pays and account balances are required to be paid at the time of service.
There are several co-pay Patient Assistance Programs available for many of the medications that we prescribe. Our staff will be happy to provide you with that information in an effort to help you keep your medication costs affordable.
Self-pay Patients
Self-pay patients are patients without any insurance coverage. Self-pay new patients are required to bring $175.00 at the initial appointment and will be billed for the remaining balance. For all follow up visits, payment is due in full at the time of service.
Out of Network
Any patient that changes insurance coverage to a healthcare plan that we do not participate with will be required to transfer care to a Rheumatologist within their plan.
