Getting Ready for Your Visit

Here is some information that may be helpful to you when seeing us for the first time, or in familiarizing yourself with our office procedures.

To make your first appointment, please call 757-220-8579. Please let our receptionist know whether you have any special needs. We will make every effort to accommodate you.

What to bring

For your first appointment, please bring:

• Your completed New Patient forms downloaded from below
• A list of All of your current medications and supplements, or the pharmacy labeled medication bottles
• Your insurance and prescription cards
• Medical records from prior visits that you think may be helpful
• A picture ID
• Co-payments (Cash, Check, American Express, Visa, Discover and MasterCard accepted)
• Completed Registration Forms

Your first appointment will last approximately one hour. Additional time may be required if x-rays and/or labs are ordered by our physicians. Please take these time requirements into account when planning your day around your appointment.

If you arrive for your new patient appointment without your new patient forms completed, we may have to reschedule your appointment.

For follow up appointments, please bring:

• Your insurance and prescription cards
• Your co-payments
• Current medication list

Please arrive 15 minutes before your scheduled appointment. If you need to cancel or change an appointment, please call 48 hours prior to the scheduled appointment. Cancellations should be no later than 24 hours prior to your appointment or you may be billed for a no show visit.

If you are over 10 minutes late for your appointment, you may be asked to reschedule.
